
Care Assessment
The care assessment is an important step in deciding what kind of help a person must receive. It is one of the first things our care managers do when a new client decides to trust us with their care or the care of a loved one. Our goal is to offer our clients a care package which covers all the aspects that improve their lives. To do so, we consider all the pertinent aspects of a person and their environment. We consider their health, personality, preferences, safety, family and much more.
We offer the care assessment for free to all our clients, since we consider it to be a vital part of the care process.
How does a Care Assessment Work
From a practical perspective, a care assessment is conducted through conversation and observation. We will speak with the person who requires care and their immediate family if the case warrants it, in order to understand what needs they have and what help we can provide. Based on the individual situation, we will get in contact with other professionals who may be involved, such as doctors and nurses.
The care assessment happens in the comfort of the person's home. It's a comprehensive process which should take around two hours. As mentioned before, we consider all the pertinent aspects of their life, in order to offer the best care we can.
The following list is by no means exhaustive, however it contains some of the things we keep in consideration when performing a care assessment:
- The personality and what the person likes and dislikes
- Other conditions and their associate medication
- Issues of mobility, hygiene, food
- Preferences in food, drinks, clothing, leisure.
- Hobbies/ activities
- The person's past life and their future goals
- When during the day is help needed the most
- Family and friends
After the Assessment
We gather as much information as possible during the assessment. This will help us formulate a care plan and choose the most appropriate carer. Circumstances can however change and so will our approach in order to provide the best service possible. We will constantly monitor and review the process by speaking with the carers, being in touch with the family, and by visiting in persons we can observe first hand the quality of care we provide.
