Returning home from the hospital is certainly a happy moment, but sometimes it presents it's own challenges. Our goal with hospital discharge care is to help you reacquire your independence and go back to living a better life.

Some of the things we can handle and help you with are medication, food preparation, mobility, hygiene or physiotherapy exercises as prescribed by your doctor.

How does Hospital Discharge Care Work?

The ideal way to address this is by preparing in advance, before returning home from the hospital. With your permission, we will coordinate with the medical team in order to better understand what your specific needs and requirements are.

Based on that, we will provide a carer who has the right training and experience to support you during the transition. Our carers can either provide whole day support, or just be present during the most challenging part of the day.
